I have this table and want to create database for it, i have created this diagram for it, could you please help me to enhance it
is this database diagram is correct?

Are Standard Test Methods specific to a Test Name, or could one Standard Test Method be used for several different Test Names?

the only possible is many method for one test name not the other way

I'd do it like this, since the test method uniquely defines the test name:
The part to the right of the QualityTestReport table remains the same.

Yes, indeed.
Check the spelling of the table name PerfromredBy - you probably meant PerformedBy
