Delete Check Boxes

Can anyone tell me how to delete the check boxes in the attached workbook. These were part of a FORM that we used , but we have edited it and no longer need the check boxes.
I have deleted the form from this example wookbook but I would like to be able to pull up the original form and just delete the check boxes.

Press Ctrl+G.
Click Special...
Select the "Objects" radio button.
Click OK.
Press Delete.
That's it!
