Create dynamic documents based on questionnaire

Weird question, but does anyone know if there is a system or app out there that can produce dynamic Word style documents (combinations of paragraphs and titles) based on the results of a questionnaire. OIW: I fill out a questionnaire, the answers I supply are evaluated by a system and a document is compiled by combining stored paragraphs and text based on my responses. Is there anything remotely like this available that I can use and test?

Are the responses numbers, or single words, or sentences, or...?

The questions can be comprehensive but the responses to those questions would be yes/no or even multiple option button choice.

I know a copy of an Acrobat form can be sent to multiple users and each can fill it in and and then submit it and all the responses are captured centrally but I've never used that feature in Acrobat so I can't be more specific.

Apparently the legal department of our company uses a data management application that has this capability. Clients would fill out a form and the responses are fed into this system which produces a compiled legal letter structured according to the individual clients responses. I never knew about this software until I did some internal investigations. Now I must just figure out if this software can be used in a different capacity.
