QAT icons
In Office 2003 one could edit the icons on the tool bar. I would routinely add commands then modify the icon to remind me what it was. Now, I have added two different commands to the QAT but both icons look the same. Is there any way to change the appearance of the icons? See pic.

For buttons associated with macros: yes, in a very limited way.
Otherwise: no.
From the look of it, I'd guess these are not associated with macros. If so, you're out of luck.

Unless I am missing the point, most button images on the QAT can be modified...

Correct: the Modify... button is only enabled for macros, NOT for built-in commands.
This is a very deplorable design decision by Microsoft. They have gotten a lot of criticism about it, but they haven't budged.

Also note that Don's screen shot is from Office 2007, while Rudi's is from Office 2010. Office 2010 allows for more Ribbon customisation that 2007. It's been a while since I tried to modify a macro button on my Excel 2007 QAT but if I remember correctly I couldn't or it was such a limited selection as to be useless. I don't remember seeing the choice that Rudi shows.
If you want to do it the hard way then with the aid of MS's Custom UI editor you can add icons to the ribbon. I have done this to put two work essential macros onto the end of Excel's Home tab but it I'm not sure I could repeat the trick now!
